Důvod a cíl výkonu There has been no obstetrical pathology found in pregnancy or by your examination to be an obstacle for delivering your child the natural way. Vaginal delivery is a natural, physiological process. 80% of deliveries are without any complications. In the remaining 20% some complications may occur which can be harmful or even life-threatening for a mother as well as an unborn child. In such case an instrumental delivery or the Caesarian section is necessary to save you or your child. We will not be able to inform you about the acute risk factors when they occur because of urgency, your stress, exhaustion or even unconsciousness. So we present these facts now. 2. Povaha a následky výkonu 3. Rizika výkonu Possible complications during the first stage of labour: These are acute situations which cannot be anticipated. Hypoxia - lack of oxygen, is the main danger threatening the child. We can detect this condition using cardiotocograph – device that detects fetal heartbeat or another device (ST segment analyzer) using special electrode attached to fetal head. Umbilical cord issue, separation of placenta and fever during delivery are the most common causes of hypoxia. Amniotic fluid embolism, eclamptic seizure and extreme hypertension belong amongst the rare causes. Nevertheless the pregnancy is immediately terminated by performing the Caesarian section in such a case. Possible complications during the second stage of labour: Umbilical cord issue continues to be the most common cause of fetal hypoxia. The fetal head is engaged into pelvis in this stage, so the Caesarian section cannot be performed without the risk of severe fetal head injury. The birth is ended quickly using obstetrical forceps or ventouse – the vacuum extraction. Larger vaginal birth injuries or fetal complications including scull fractures, brain hemorrhage, head and upper extremities neural damage, the skin separated from the scull occur in 6-8% of instrumental deliveries. Episiotomy – incision of the perineum is performed during instrumental deliveries as well as deliveries of bigger babies to prevent uncontrolled tearing of perineum and vaginal wall which can damage the anal sphincter or rectal wall. A general anesthesia may be required to suture the larger injuries. You will be given laxatives so the healing process is eased by the loose stools in case of anal sphincter or rectal wall injury. A need for temporary stoma – surgically created bowel duct through the abdominal wall is extremely rare. Possible complications in the third stage of labour: They occur mostly due to excessive bleeding after the child is born. It can be caused by vaginal or perineal injury, insufficient shrinking of the uterus or withholding of placenta or its part. Huge blood loss may lead to shock development. We use surgical interventions to eliminate the causes of bleeding such as suturing the vaginal birth injury, evacuating withheld placental parts or the whole placenta out of the uterine cavity. Excessive bleeding which cannot be solved another way may lead to removal of the uterus and blood transfusion. Perineal, urinary bladder or womb infection may occur while staying in the hospital few days after you deliver. Such an infection is treated with antibiotics. 4. Hospitalizace a omezení, doporučení ve způsobu života Hospitalization and recuperation: It is possible to discharge a healthy neonate 72 hours after the birth according to the Neonatology society. Hospitalization can be longer on account of the mother or the infant. Rooming-in is applied in the puerperium department in our hospital which means that the child and his mother stay in the same room. When the child cannot be discharged his mother usually stays in the hospital with him to continue breastfeeding. Alternativy výkonu Alternatives: The alternative to a natural childbirth is the Caesarian section. It is considerate a severe abdominal operation with strict indications. We are not able to perform the Caesarian section on demand. The risk of thrombosis and pulmonary embolism increase six times while the Caesarian section is performed.
