In order to receive radioactive materials from CERN, each Institute must have a valid licence registered
in our database1, together with full details of the Institute’s current Radiation Safety Officer. If your
institute does not have a licence, or the licence is out of date and/or your RSO is not in our database, we
kindly request you to complete and return the following questionnaire, together with a copy of the valid
licence.
Please note that until this information is corrected/completed we are unable to ship radioactive material to
your institute.
Preferably we would like the certificate in English or French, but if this is not possible then we require
at least a summary in English or French, specifying the beginning and end dates (if any) and any
restrictions. We prefer to receive the Licence and the questionnaire by email (mailto:[email protected]), in PDF format, if not by post to the following address:

Reasoning:
This Decree,I issue accordingto your Application submittedin December16, 1998and
basedon the results ofthe control visit ofyour laboratoryin January29, 1998performed
byan inspectorofthe StateOffice for Nuclear Safety,the submitteddocumentationmeet
demandsof §13, and the SupplementI of the sited Act. During the control of your
laboratory no deficiencieswere found that would precludeissuing of this Decreeof the
handling of open,closedradionuclideemittersand generatorsof radiations.
Edification:
According to §61 of the Act 71/1967 on administrativeproceedings,it is possible to
protest againstthis decisionto the presidentof the StateOffice for Nuclear Safety.

Odùvodnìní:
Rozhodnutí vydávám na Vaši žádost ze dne 16.12.1998a na základì výsledku kontrolní
návštìvy pracovištì dne 29.1.1998 provedenéinspektorem SÚJB.Pøedložená
dokumentace
splòuje požadavky § 13 a pøílohyI citovanéhozákona.Šetøenímna pracovišti samémnebyly
zjištìny závažnénedostatky, které by bránily vydání rozhodnutí pro používání otevøených,
uzavøenýchradionuklidových záøièùa generátorùzáøení.
Pouèení:
Podle § 61 zákonaè.71/1967Sb., o správnímøízení,lze proti tomuto rozhodnutí podat do 15
