Since then, dozens of publications have contributed to provide a clearer picture of the real impact of AF. In the last couple of years, two systematic reviews of the literature on global epidemiology of AF have provided concrete evidence of our increasing iincidence nci c deenc ncee of tthis his knowledge in this area.2, 3 Both reviews highlighted the growing prevalence andd inci arrhythmia globally, but also called attention to the limited information on AF epidemiology in developing de eve velo lopi lo ping pi ng ccountries. ount nttri ries e . Using evidence from 184 different publications, they concluded that in 2010 approximately 33.5 million individuals have AF worldwide, with close to 5 million new cases occurring each year; that both the prevalence and incidence of the disease have increased meaningfully over the last 2 decades; and that, as a consequence, the life years lost to AF also have increased, with a striking doubling of the AF-associated mortality in the study period. 2 DOI: 10.1161/CIRCULATIONAHA.113.007482 Additionally, this study confirms previous observations, including the direct association between older age and AF burden, and the higher incidence and prevalence of AF in men than in women.5 Of note, the authors observed marked between-region variability in prevalence and incidence of AF, with the lowest burden in the Asia Pacific region, and the highest in North America. The major strengths of the manuscript by Chugh and colleagues, compared with previous efforts to review the global epidemiology of AF, are the strict methodological approach and the use of the Global Burden of Disease framework. This approach standardizes the pooling of Downloaded from http://circ.ahajournals.org/ by guest on October 12, 2016 evidence from different studies, makes explicit assumptions about the modeling process, and, importantly, facilitates comparing the burden attributable to AF across regions and with that of other diseases. Nonetheless, some caveats need to be mentioned. North America had the highest burden of AF, while the Asia Pacific region, particularly Japan, the Koreas and China, had the lowest. The factors underlying these differences are not known precisely, but several explanations can be offered. First, higher detection rates of AF in North America than in other regions, even if true rates were similar, would lead to this observation. Second, diversity in the prevalence of risk factors for AF, such as obesity or hypertension, could be responsible for Downloaded from http://circ.ahajournals.org/ by guest on October 12, 2016 between-region variability. Finally, differences in the underlying risk due to genetic factors might justify this regional heterogeneity. Several previous studies, predominately in the United States, reported lower risk of AF in non-whites compared to whites,9, 10 which could coould ould d translate tra rans ra nsla ns late la t to te to higher risk of AF in North America than in Eastt Asia or Sub-Saharan Africa. The aging of the population, the improved survival of individuals with coronary artery disease and heart failure, major risk factors for the arrhythmia, and the growing ability to identify AF are factors that have contributed to the upward trend in AF burden over the last decades and will continue to do so, particularly in developing regions. Furthermore the obesity epidemic in the United States and other countries may contribute to further rises in the incidence of AF. These trends would be less worrisome if we had established interventions for primary prevention of AF. Unfortunately, we lack strong evidence supporting any specific intervention 4 DOI: 10.1161/CIRCULATIONAHA.113.007482 for this purpose, beyond appropriate control of cardiovascular risk factors. More research aimed to develop and test strategies for the primary prevention of AF is desperately needed. Where do we go next? The work by Chugh and colleagues underscores existing gaps in our knowledge on the global epidemiology of AF and, therefore, provides a blueprint for action. First, more research on the prevalence and incidence of AF in developing countries, using standardized methods, is necessary in order to have a more complete and accurate picture of the AF burden. Second, Downloaded from http://circ.ahajournals.org/ by guest on October 12, 2016 studies of the effect of AF on mortality, stroke, and other complications need to be conducted, mostly in developing regions, to understand the impact of the arrhythmia and the associated treatment associated with AF, reatment needs. Hlavní silnou stránkou studie Chugha a spol. je, v porovnání s předešlými pokusy o průzkum celosvětové epidemiologie FS, přesná metodika a využití struktury studie Global Burden of Disease. Tento postup standardizuje získávání důkazů z různých studií, činí otevřeným proces modelování dat a usnadňuje srovnání zátěže FS v jednotlivých regionech a v porovnání s dalšími chorobami. Nicméně je třeba uvést některé námitky. Zaprvé, ačkoliv tento systematický přehled zahrnoval > 180 původních publikací, < 30 % z nich se týkalo oblastí mimo Severní Ameriku a Západní Evropu. Důležitým nálezem této části studie Global Burden of Disease je výrazná heterogenita odhadů prevalence a incidence mezi jednotlivými regiony. Severní Amerika má nejvyšší výskyt FS, zatímco oblast Asie-Pacifik (tedy jižní + jihovýchodní + východní Asie + Oceánie) a to zejména Japonsko, Severní a Jižní Korea a Čína mají nejnižší výskyt FS. Faktory, které vedou k těmto rozdílům, nejsou přesně známé, ale je možno nabídnout několik vysvětlení. Zaprvé, lepší diagnostické možnosti a vyšší záchyt FS v Severní Americe než v jiných oblastech by vedly k obdobnému pozorování, i kdyby byl výskyt ve všech oblastech stejný. Zadruhé, za variabilitu ve výskytu mezi jednotlivými oblastmi by mohly být zodpo- klíčové přednášce v Massachusetts Medical Society v roce 1997 označil dr. Eugene Braunwald fibrilaci síní (FS) za nově se objevující epidemii kardiovaskulárního onemocnění.1 V té době byly informace o zátěži FS—prevalenci, incidenci a prognóze—v běžné populaci omezené na několik málo epidemiologických studií, většinou provedených ve Spojených státech a západní Evropě. Od té doby přispěly desítky publikací k vytvoření jasnější představy o skutečném dopadu FS. Tyto publikace potvrdily předpověď dr. Braunwalda a ukázaly, že FS je nejčastější setrvalou arytmií v klinické praxi a rovněž zásadním problémem pro veřejné zdravotnictví. V posledních několika letech poskytly dva publikované systematické přehledy literatury týkající se celosvětové epidemiologie FS konkrétní důkazy pro naše zlepšující se znalosti v této oblasti.2,3 Obě práce poukázaly na narůstající prevalenci a incidenci této arytmie v celosvětovém měřítku, ale rovněž na omezené informace o epidemiologii FS v rozvojových zemích. Jeden z těchto přehledných článků například prezentoval pouze 6 publikací týkajících se incidence FS mimo Spojené státy a západní Evropu.2 Editorial se vztahuje k abstraktu článku na straně 23 V tomto čísle Circulation se Chugh a spol. pokusili poskytnout úplnější odhad celosvětové zátěže FS.4 Studie Global Burden of Disease 2010 byl projekt založený na mezinárodní spolupráci systematicky hodnotící celosvětová data týkající se všech onemocnění a poranění. V rámci této studie přezkoumali autoři literaturu týkající se epidemiologie FS publikovanou mezi roky 1980 a 2010 s cílem zjistit incidenci a prevalenci FS a zátěž spojenou s tímto onemocněním. Autoři použili data ze 184 různých publikací a zjistili, že v roce 2010 má na celém světě přibližně 33,5 milionů nemocných FS a ročně se objeví téměř 5 milionů nových případů. Konečně, příčinou takové heterogenity mohou být genetické rozdíly, které mohou určovat skryté riziko FS. Kromě toho, že článek publikovaný Chughem a spol. přinesl nejnovější souhrn celosvětového výskytu FS, naznačuje, že výskyt FS bude nadále narůstat. Stárnutí populace, lepší přežívání nemocných s ischemickou chorobou srdeční a srdečním selháním, což jsou hlavní rizikové faktory pro vznik této arytmie, a zároveň zlepšující se možnosti diagnostiky FS jsou důležité faktory, které přispěly k vzestupnému trendu ve výskytu FS v posledních dekádách, a které budou nadále k tomuto trendu přispívat, a to zejména v rozvojových zemích. Navíc epidemie obezity ve Spojených státech a dalších zemích může také přispět k dalšímu nárůstu incidence FS. Tento trend bude méně znepokojivý, pokud se nám podaří intervenovat v oblasti primární prevence FS. Bohužel však nemáme dostatečně silné důkazy, že by jakákoliv intervence kromě dostatečné kontroly rizikových faktorů kardiovaskulárních onemocnění, byla v tomto směru účinná. Je tedy opravdu potřeba věnovat se více vývoji strategií pro primární prevenci FS a ověřování jejich účinnosti. Kam směřujeme? Práce Chugha a spol. zdůrazňuje existující nedostatky v našich znalostech o celosvětové epidemiologii FS, a tudíž poskytuje plán pro další kroky. Zaprvé, je potřebný další výzkum prevalence a incidence FS v rozvojových zemích s použitím standardizovaných metod, abychom získali kompletnější a přesnější obraz o výskytu FS. Zadruhé, je nutné se nadále věnovat výzkumu dopadu FS na mortalitu, výskyt cévních mozkových příhod a dalších komplikací, a to zejména v rozvojových zemích, abychom lépe pochopili důsledky této arytmie a potřebu léčby tohoto onemocnění. Zatřetí, je potřeba lépe popsat invaliditu spojenou s tímto onemocněním, aby výpočet parametru DALY v důsledku FS lépe odrážel opravdovou zátěž způsobenou touto arytmií. Konečně, jak již bylo zmíněno výše, vývoj intervencí a strategií na úrovni primární 21a22_E_Alonso.indd 22 prevence je zásadní, pokud chceme snížit globální výskyt FS v rozvinutých a rozvojových zemích. Závěrem je třeba dodat, že Chugh a spol. potvrdili v celosvětovém měřítku prorocká slova dr. Braunwalda vyslovená před téměř 20 lety. Jejich článek je výzvou pro všechny vědce, kliniky a osoby ovlivňující zdravotnickou péči, kteří mají zájem na snížení výskytu kardiovaskulárních onemocnění. Zdroje financování Práce dr.
