Resuscitation is challenging but achievable, and relies in part on the chain of survival which includes early arrest recognition and emergency activation, early cardiopulmonary resuscitation (CPR), early defibrillation, expert advanced life support and integrated post-resuscitation care 3. Specifically, early CPR performed by laypersons can double the chances of survival and provides an important foundation for subsequent links in the chain of survival 4. Some experimental and observational studies suggest that chest compression only CPR may provide more benefit than traditional CPR and yet other studies indicate that the type-specific benefits of CPR may depend on patient or circumstantial factors 7-12. In randomized trials comparing dispatcher CPR instruction that consisted of chest compression alone or compression plus rescue breathing, results have not been conclusive 5, 13, 14. A meta-analysis of these randomized trials restricted to those patients with a cardiac aetiology suggested a short-term survival benefit of chest compression alone 15, 16. However the ability to apply type-specific CPR according to arrest aetiology imposes an 3 DOI: 10.1161/CIRCULATIONAHA.112.124115 artificial selection that cannot be readily achieved in the field by laypersons or by dispatchers17. We undertook long-term follow-up of subjects enrolled in 2 randomized trials comparing dispatcher CPR instruction to determine if random allocation of type-specific CPR was associated with long-term survival13, 14, 18. The potential benefits of chest compression alone might be amplified following hospital discharge. This premise is derived from the appreciation that arrest survivors with underlying cardiac aetiology have a better long-term prognosis than those with non-cardiac aetiology19. In addition, some evidence suggests that brain recovery in Downloaded from http://circ.ahajournals.org/ by guest on October 12, 2016 particular may benefit from the chest compression alone strategy and that the effects of brain recovery might not be fully evident until more protracted follow-up20. The respective review boards approved the two trials including long-term surveillance to determine post discharge survival. The studies are registered in clinicaltrials.gov (NCT00219687) and in Karolinska Clinical Trials Registry (kctr.se;CT20080012) respectively Subjects and settings 4 DOI: 10.1161/CIRCULATIONAHA.112.124115 Eligibility and enrollment have been described in detail previously. Briefly subjects were eligible for the DART study if they were unconscious and not breathing normally and bystander CPR was not ongoing and the bystander was willing to receive instruction. Subjects were enrolled from June 1, 2004 to April 15, 2009. Dispatchers attempted to exclude those with cardiac arrest due to obvious trauma, drowning, or asphyxiation (from strangulation or suffocation), as well as patients who were under 18. In TANGO, subjects were eligible if the collapse was witnessed (seen or heard), and the ubject was unconscious and not breathing normally. In TANGO communities, emergency medical services are provided by paramedics and nurses 22. The EMS in both study settings follows the core resuscitation strategy detailed by International Guidelines 23, 24. Study Intervention Once determining that a case was eligible, dispatchers opened a sealed envelope (DART) or pulled a paper strip (TANGO) that contained type-specific CPR instruction. Chest compression alone instruction consisted of repeated cycles of compressions without instruction for rescue 5 DOI: 10.1161/CIRCULATIONAHA.112.124115 breathing. Chest compression plus rescue breathing instruction consisted of 2 initial rescue breaths followed by alternating 15 chest compressions13, 14. Covariates Data was collected about patient, circumstance, care, and outcome characteristics according to the Utstein data elements25. The study used information available from dispatch, EMS, hospital, and death certificate records. In the DART study, vital status was determined by using State Vital Records and the Social Security National Death Index. To compare long-term survival, we first used Kaplan Meier product-limit method to estimate survival at 1, 3 and 5 years according to randomization assignment. Comparison of survival curves used both log-rank test and Tarone-Ware test 18. We also used Cox multivariable regression employing Efron method to test the association between the intervention and survival adjusting for potential confounders including age, gender, initial rhythm, etiology of the arrest, witnessed status, location of arrest, interval from call receipt to EMS scene arrival, and study. 6 DOI: 10.1161/CIRCULATIONAHA.112.124115 The proportional hazards assumption was evaluated graphically and tested based on Schoenfeld residuals. Instead of excluding cases with missing covariates, we performed multiple imputation to incorporate all subjects in the fully-adjusted model 26. We conducted a sensitivity analysis excluding those with missing covariates from the multivariable model. We performed secondary analyses that included an efficacy comparison restricted to those who actually received bystander CPR. We also performed subgroup analyses defined by arrest etiology, presenting arrest rhythm, witnessed status, and EMS response interval among witnessed arrests. Al gn ll tests All analyses were performed using STATA 11.2/SE software (College Station, Texas, US) Results The original TANGO trial included 1276 subjects while the original DART trial included 1941 subjects. Of those, 2496/3217 (78%) were included in the current study (Figure 1). The primary reason for exclusion was the London site (n=655) where long-term vital status could not be determined. Although overall cases from the London site were older, less often had witnessed collapse, and less frequently presented with a shockable rhythm, these characteristics were 7 DOI: 10.1161/CIRCULATIONAHA.112.124115 distributed equally between the intervention groups within the London site. Of the 2496 subjects included in the current investigation, 1243 (50%) were randomized to chest compression alone and 1253 (50%) were randomized to chest compression plus rescue breathing. Baseline characteristics were similar between the two CPR groups (Table 1). Overall, men were twice as common as women. The median age was 66 years [25th, 75th percentiles, (55,77)]. About three-quarters suffered an arrest due to a cardiac aetiology while about one-third presented with a shockable arrest rhythm. The beneficial mortality association was similar when those with missing covariate status were excluded from the model (HR=0.91 [0.84-0.99, p=0.03). When stratified by the early and late phase of follow-up (<= day 30 and subsequent to day 30), chest compression alone compared to chest compression plus rescue breathing was associated with a lower risk of death during the early phase (adjusted HR=0.90; 95% CI [0.83-0.98], p=0.02). The intervention was not associated with mortality risk during the late phase (adjusted HR=0.99; 95% CI [0.62-1.58], p=0.99). During the study, 1918/2496 (77%) patients progressed through instructions and received the intended CPR. When restricting to this efficacy cohort, chest compression only was 8 DOI: 10.1161/CIRCULATIONAHA.112.124115 associated with a lower risk of death after adjustment for potential confounders (adjusted HR=0.90; 95% CI [0.82-0.99], p=0.03). We did not observe strong evidence of subgroup differences as none of the interaction terms between intervention status and the subgroup of interest attained statistical significance (Figure 3). We did however observe a lower risk of death among those with an arrest due to a cardiac etiology (HR=0.86, 95% CI [0.77-0.97], p=0.01) and those with a witnessed arrest (HR=0.89, 95% CI [0.81-0.99], p=0.03). Prior investigations have restricted or stratified findings according to arrest aetiology because the survival effects of type-specific CPR may depend upon etiology 7, 8, 11, 27, 28. Although these stratified findings provide useful mechanistic insights, dispatchers are challenged to quickly and correctly determine etiology, making etiology-based instruction impractical for real-world implementation 17. Moreover, no study has evaluated the long-term outcomes of these patients. Although survival to hospital discharge is a clinically meaningful outcome, the short-term survival effects of type-specific CPR could be attenuated or amplified during 9 DOI: 10.1161/CIRCULATIONAHA.112.124115 convalescence and long-term follow-up. The current results provide important evidence that chest compression alone instruction can achieve better overall prognosis and should be considered the instructional approach for nearly all adult patients for whom dispatchers suspect cardiac arrest. Based on the survival curve and short-term and long-term stratified analyses, we observed that the survival benefit of chest compression alone appears to be attributable to an early survival differential that persists over subsequent years of follow-up. Importantly, we did not observe evidence of harm among those for whom oxygenation and ventilation might in theory be more important such as non-cardiac aetiology or unwitnessed arrest (Figure 3). The study has limitations. The original studies were conducted independently and were 10 DOI: 10.1161/CIRCULATIONAHA.112.124115 not originally designed to ascertain long-term outcome. Common entry criteria and the randomization approach help protect against bias and enable combination of the study data. Each study used national and provincial death registries to ascertain mortality. Adjustment for study did not change the results and the results were similar between the two studies. Although we observed survival differences, we were not able to ascertain functional status or quality of live during long-term follow-up though favourable functional status has been associated with better long-term prognosis in the general population 36. The results are specific to dispatcher-assisted layperson CPR though they provide a useful context to consider layperson CPR training and guidelines. Specifically, some evidence suggests that laypersons are especially challenged to perform effective rescue breathing even after training38. Thus the results of the current study support the 2010 Guidelines that prioritize chest compressions regardless of training status or dispatcher assistance. Laypersons are directed to proceed to ventilations only if proficient in the technique of rescue breathing. In conclusion, the findings provide strong support for long-term mortality benefit of 11 DOI: 10.1161/CIRCULATIONAHA.112.124115 dispatcher CPR instruction strategy consisting of chest compression alone rather than chest compression plus rescue breathing among adult cardiac arrest patients requiring dispatcher assistance. Emergency dispatchers have a vital role in resuscitation and community stakeholders should leverage this important role to increase early arrest recognition and effective layperson CPR and in turn improve survival following cardiac arrest. Bystanders can proceed with chest compression alone approach with the appreciation that this strategy on average provides optimal long-term survival benefit. Naším předpokladem bylo, že dispečerem řízená KPR zahrnující pouze kompresi hrudníku bude mít lepší celkovou dlouho-dobou prognózu v porovnání s kompresí hrudníku v kombinaci s umělým dýcháním. Metody a výsledky—Jedná se o retrospektivní analýzu 2 randomizovaných studií, které porovnávaly krátkodobé přežití, kdy dispečerovou instrukcí byla pouze komprese hrudníku ve srovnání s kompresí hrudníku a umělým dýcháním. Dlouhodobé přežití bylo zjišťováno s použitím Národního a státního registru úmrtí do 31. července 2011. Ke zhodnocení přežití podle typu instrukce KPR jsme použili Kaplan-Meierovu metodu a metodu Coxovy regrese. Do studie bylo zařazeno 2 496 pacientů, z toho 1 243 (50%) bylo randomizováno k samotné kompresi hrudníku a 1 253 (50%) ke kompresi hrudníku a umělému dýchání. Základní charakteristiky nemocných byly podobné v obou randomizovaných skupinách. Během 1 153,2 osobo-roků následného sledování jsme zaznamenali 2 260 úmrtí a 236 dlouhodobých přežití. Samotná komprese hrudníku měla v porovnání s kompresí hrudníku v kombinaci s umělým dýcháním po statistickém zhodnocení nižší riziko úmrtí (hazard ratio, 0,91; 95% interval spolehlivosti, 0,83–0,99; p = 0,02). Závěr—Naše výsledky jasně ukazují snížení dlouhodobé mortality, pokud dispečer dává pokyny pouze k samotné kompresi hrudníku ve srovnání s kombinací komprese hrudníku a umělého dýchání. Samotná komprese hrudníku je jednodušší a rychleji zahájena, proto může zajistit časnou KPR u většího množství osob se srdeční zástavou.5 Nicméně účinnost samotné komprese hrudníku ve srovnání s tradiční KPR zajišťovanou laiky je diskutabilní.6 Některé experimentální a observační studie naznačují, že samotná hrudní komprese může být prospěšnější než tradiční KPR a jiné ukazují, že prospěch z typu KPR závisí na stavu pacienta a daných okolnostech.7–12 V randomizovaných studiích porovnávajících pokyny dispečera KPR, které obsahovaly samotnou kompresi hrudníku proti kombinaci komprese hrudníku a umělého dýchání, nebyly výsledky přesvědčivé.5,13,14 Meta-analýza těchto randomizovaných studií, omezená jen na pacienty s kardiální etiologií zástavy, ukazovala zlepšení rdeční zástava mimo nemocnici je celosvětově hlavním důvodem úmrtí pacientů.1,2 Resuscitace je výzvou, která je však dosažitelná a závisí zčásti na jednotlivých složkách tzv. řetězu přežití. Ten zahrnuje časné rozpoznání srdeční zástavy, aktivaci záchranné služby, časnou kardiopulmonální resuscitaci (KPR), časnou defibrilaci, pokročilou profesionální životní podporu a integrovanou poresuscitační péči.3 Obzvláště časná KPR prováděná laiky může zdvojnásobit šance na přežití a zajišťuje důležitý základ pro následné články řetězu přežití.4 V terénu většinou dostane laickou KPR před příjezdem profesionálních záchranářů méně než polovina osob se srdeční zástavou.4 Souhrn pro klinickou praxi na straně 94 Ke zlepšení a podpoře laické KPR bylo již použito mnoho rozdílných přístupů. Circulation is available at http://circ.ahajournals.org DOI: 10.1161/CIRCULATIONAHA.112.124115 89 90 Circulation říjen, 2013 krátkodobého přežití při použití samotné komprese hrudníku.15,16 Nicméně použití specifického typu KPR podle patogeneze zástavy přináší umělou selekci, která může být v terénu pro laiky či dispečery obtížně zvládnutelná.17 Hodnotili jsme dlouhodobé sledování pacientů zařazených do 2 randomizovaných studií porovnávajících dispečerovy pokyny KPR, abychom zjistili, zda je náhodné přidělení určitého typu KPR spojeno s dlouhodobým přežitím.13,14,18 Potenciální přínos samotné hrudní komprese může být zvýšen následně po propuštění z nemocnice. Toto předpokládáme s vědomím, že pacienti, kteří přežili zástavu kardiální etiologie, mají lepší dlouhodobou prognózu než ti s nekardiální etiologií zástavy.19 Navíc, některá zjištění ukazují, že konkrétně zotavení mozku může profitovat pouze ze samotné hrudní komprese a efekt zotavení mozku je plně patrný až při delším sledování pacienta.20 Proto jsme předpokládali, že pokyny dispečera sestávající pouze ze samotné komprese hrudníku mají ve srovnání s kompresí hrudníku a umělým dýcháním lepší celkovou dlouhodobou prognózu. Metody Design studie V naší studii se jednalo o retrospektivní analýzu skupiny pacientů, kdy jsme desing studie převzali od DART studie (Dispatch Assisted Resuscitation Trial) a od švédské randomizované studie nazvané KPR řízená TANGO- telefonem (TANGO).13,14 Tyto 2 studie porovnávaly, zda se přežití do doby propuštění z nemocnice lišilo u pacientů se srdeční zástavou v terénu podle toho, zda vybraný dispečer řídil KPR pouze jako samotnou kompresi hrudníku nebo kompresi hrudníku spojenou s umělým dýcháním. Příslušná hodnotící komise schválila tyto 2 studie včetně dlouhodobého sledování ke zjištění přežití po propuštění z nemocnice. Studie jsou v tomto pořadí registrovány na http://clinicaltrials.gov (NCT00219687) a v Karolinska Clinical Trials Registry (http://www.kctr.se; CT20080012). Pacienti a podmínky Kritéria studií a samotné zařazení pacientů bylo již detailně popsáno dříve. Ve zkratce, do studie DART byli zařazeni pacienti, kteří byli v bezvědomí a měli zástavu dechu, u kterých ještě nebyla zahájena laická resuscitace a přítomný laik byl ochoten přijímat pokyny od dispečera. Pacienti byli zařazováni od 1. června 2004 do 15. dubna 2009. Dispečeři vyřadili pacienty se srdeční zástavou zapříčiněnou traumatem, utonutím nebo udušením (škrcením nebo vdechnutím cizího předmětu) a současně pacienty mladší 18 let věku. Zároveň ze studie DART byli také vyřazeni pacienti, kteří pocházeli z oblasti Londýna, neboť zde nebylo možné zajistit jejich dlouhodobé sledování. Do studie TANGO byli zařazeni pacienti, kteří byli v bezvědomí se zástavou dechu, u jejichž kolapsu byli svědci, kteří událost viděli nebo slyšeli. Pacienti do studie TANGO byli zařazováni od 1. února 2005 do 31. ledna 2009. Ze studie byli vyřazeni nemocní se srdeční zástavou způsobenou traumatem, obstrukcí dýchacích cest, utonutím nebo intoxikací a mladší 18 let. V protokolech obou studií byla odborná pomoc aktivována vytočením čísla tísňové linky s následným kontaktem nouzového dispečera. V oblastech s pacienty ze studie DART, je zdravotnický záchranný systém 2 stupňový. První stupeň představuje záchranu základních životních funkcí a je zajištěn vyškoleným lékařským technikem – školení hasiči. Druhý stupeň představuje pokročilou zdravotnickou pomoc zajištěnou paramediky.21 V oblastech ze studie TANGO je záchranný zdravotnický systém zajišťován paramediky a zdravotními sestrami.22 V obou studiích se záchranné zdravotnické systémy řídí resuscitační strategií detailně popsanou v mezinárodních doporučeních.23,24 Studijní intervence Pokud se jednalo o vhodný případ do studie, dispečer otevřel zalepenou obálku (studie DART) nebo vylosoval proužek papíru (studie TANGO), na kterém byl popsán konkrétní typ KPR. Samotná komprese hrudníku představovala opakované stlačování hrudního koše bez pokynů k umělému dýchání. Pokyny komprese hrudníku v kombinaci s umělým dýcháním představovaly 2 zahajující vdechy, po kterých následovalo 15 rytmických stlačení hrudníku.13,14 Další údaje Data o pacientech, okolnostech, péči a výsledcích byla shromažďována dle Utsteinského protokolu.25 Ve studii byly využity informace získané z dispečinku zdravotnického záchranného systému, nemocnice a úmrtních listů. Výsledky léčby Cílovým bodem naší studie bylo, zda nemocný přežil či nikoli. Ve studii DART bylo toto zjišťováno ze Státních záznamů o narození a úmrtí a z Národního registru úmrtí sociálního zabezpečení. Ve studii Tango byly tyto informace získávány z Národního registru srdečních zástav a Národního registru osobních informací. Informace o přežití či úmrtí byly kompletovány z příslušných národních registrů do 31. července 2011, a to bez znalosti randomizace. Statistická analýza K porovnání charakteristik podle přidělené intervence (samotná komprese hrudníku nebo komprese hrudníku v kombinaci s umělým dýcháním) jsme použili Pearsonův χ2 test pro kategorické proměnné a neparametrický Wilcoxonův test pro kontinuální proměnné. K porovnání dlouhodobého přežití jsme nejprve použili Kaplan-Meierovu metodu hodnocení přežití v 1., 3. a 5. roce podle randomizace. K porovnání křivek přežití bylo využito log-rank a Tarone-Ware testu.18 K testování vztahu mezi intervencí a přežitím s adjustací na okolnosti, které mohou situaci ovlivnit, jako je věk, pohlaví, vstupní srdeční rytmus, patogeneze srdeční zástavy, svědectví, místo srdeční zástavy, časový interval od přijetí hovoru do příjezdu záchranné služby, jsme použili Coxovu multivariační regresi s využitím Efronovy metody. Proporcionální riziko bylo vyhodnoceno graficky a testováno na základě Schoenfeldových reziduí. Namísto vyřazení případů, kde jsme přesně neznali všechny sledované proměnné, jsme použili metodu mnohonásobné imputace s cílem zařadit do upraveného modelu všechny případy.26 Analyzovali jsme senzitivitu s vyloučením těch případů, kde chyběly proměnné multivariačního modelu. Vytvořili jsme sekundární analýzy k porovnání účinnosti KPR, omezené na pacienty, kteří byli doopravdy resuscitováni kolemjdoucími. Dále jsme hodnotili intervenci dle časového úseku následného sledování. Rozdělili jsme ho na časnou fázi prvních 30 dní po zástavě a pozdní fázi dalšího období po těchto 30 dnech. Dále jsme vytvořili analýzu podskupin, které byly definovány patogenezí zástavy, přítomným srdečním rytmem, svědectvím zástavy a časovým intervalem mezi svědectvím zástavy a zásahem záchranné služby. Hodnotili jsme rozdíly v typu intervence ve vztahu k podskupinám určeným sledovanými proměnnými. Nakonec jsme hodnotili vztah mezi intervencí a přežitím pacientů v primární skupině Utsteinské klasifikace – oběhová zástava srdečního původu se svědky a přítomností rytmu, který lze defibrilovat a laickou KPR. Všechny testy byly dvoustranné. Za statisticky významnou byla považována hodnota p ≤ 0,05. Všechny analýzy byly vyhotoveny za použití STATA 11.2/SE softwaru (StataCorp, College Station, TX). Výsledky Do původní studie TANGO bylo zahrnuto 1 276 pacientů, zatímco do původní studie DART 1 941. Do naší studie bylo Dumas a kol. Vyřazeni byli zejména pacienti z oblasti Londýna (n = 655), kde nemohlo být zajištěno dlouhodobé sledování. Celkově byly případy z londýnské oblasti staršího data, bylo zde méně kolapsů se svědky a méně zástav s rytmem, který lze defibrilovat. Tyto charakteristiky v rámci londýnské oblasti byly rovnoměrně rozděleny mezi intervenované skupiny. Z 2 496 pacientů zařazených do naší studie bylo 1 243 (50%) randomizováno k samotné kompresi hrudníku a 1 253 (50%) ke kompresi hrudníku v kombinaci s umělým dýcháním. Základní charakteristiky pacientů byly v obou skupinách obdobné (tabulka 1). Mužů bylo dvakrát více než žen. Medián věku byl 66 let (25., 75. percentil [55,77]). Okolo 3/4 pacientů mělo zástavu kardiální etiologie, přibližně 1/3 měla při zástavě srdeční rytmus, který bylo možno defibrilovat. Během 1 153,2 osobo-roků sledování jsme zaznamenali 2 260 úmrtí. Celková míra přežití byla 11% (9,8–12,2) v 1. roce, 10,6% (8,9–11,3) ve 3. roce a 9,4% (8,3–10,6) v 5. roce sledování. Kaplan-Meierovy křivky srovnávající 2 strategie KPR prováděné laiky ukázaly lepší přežití u těch pacientů, kteří byli randomizováni k samotné kompresi hrudníku ve srovnání s kombinací komprese hrudníku a umělého dýchání. (Log-rank test p = 0,03, Tarone–Ware test p = 0,009) (obrázek 2). V multivariačním modelu Coxovy regrese byl proporcionální odhad rizika uspokojivý (χ2 0,18, df = 1, p = 0,67). Skupina randomizovaná k samotné kompresi hrudníku měla při zohlednění eventuálních zkreslujících okolností nižší riziko úmrtí (upravené hazard ratio [HR] 0,91; interval spolehlivosti [CI] 95%, 0,83–0,99; p = 0,02) (tabulka 2). Zlepšení mortalitních dat bylo podobné i pokud jsme pacienty, u kterých chyběly informace o okolnostech zástavy, ze statistického modelu vyloučili (HR, 0,91; 95% CI, 0,84–0,99; p = 0,03). Jestliže následné sledování ve studii rozdělíme na časnou a pozdní fázi (období prvních 30 dnů a období po těchto 30 dnech), měla samotná komprese hrudníku ve srovnání s kombinací komprese hrudníku a umělého dýchání nižší riziko úmrtí v časné fázi (HR, 0,90; 95% CI, 0,83–0,98; p = 0,02). Intervence nebyla spojována s rizikem mortality během pozdní fáze (HR, 0,99; 95% CI, 0,62–1,58; p = 0,99). Ve studii bylo 1 918/2 496 (77%) pacientů, u kterých bylo postupováno dle pokynů a obdrželi určenou KPR. Pokud se omezíme na tuto skupinu pacientů, pak samotná Tabulka 1. Nezjistili jsme žádný významný rozdíl ve sledovaných podskupinách, protože žádná interakce mezi intervencí a sledovanou podskupinou nedosáhla statistické významnosti (obrázek 3). Nicméně jsme zjistili nižší riziko úmrtí u těch pacientů, kteří měli srdeční zástavu kardiální etiologie (HR, 0,86; 95% CI, 0,77–0,97; p = 0,01) a také u pacientů, u jejichž srdeční zástavy byli svědci (HR, 0,89; 95% CI, 0,81–0,99; p = 0,03). Pokud jsme se soustředili na primární Utsteinskou skupinu – oběhová zástava srdečního původu se svědky a přítomností rytmu, který lze defibrilovat (n = 713) – randomizace k samotné kompresi hrudníku měla nižší riziko úmrtí než Tabulka 2. Diskuse V tomto rozsáhlém projektu zahrnující 2 studie porovnávající dispečerské pokyny KPR, měli pacienti se srdeční zástavou, kteří byli randomizováni pouze k samotné kompresi hrudníku lepší dlouhodobé přežití než ti, kteří byli randomizováni ke kompresi hrudníku a umělému dýchání. Předchozí studie hodnotily výsledky v závislosti na patogenezi zástavy, protože přežití při různém typu KPR může na patogenezi zástavy záviset.7,8,11,27,28 Ačkoli takto získané výsledky přinášejí jistě přínosný mechanistický pohled, dispečeři jsou nuceni rychle a správně posoudit patogenezi zástavy a dle toho správně řídit KPR v daném konkrétním případě, což není vhodné pro obecné praktické doporučení.17 Mimo to, zatím žádná studie nehodnotila dlouhodobé výsledky těchto pacientů. Přestože přežití do propuštění z nemocnice je klinicky významný výsledek, účinky typově specifické KPR na krátkodobé přežití mohou být oslabeny nebo zesíleny během rekonvalescence a dlouhodobého sledování. Současné výsledky potvrzují, že samotná komprese hrudníku může zajistit lepší dlouhodobou prognózu a měla by být použita téměř u všech dospělých pacientů, u kterých mají dispečeři podezření na srdeční zástavu. Na základě křivky přežití a krátkodobých i dlouhodobých stratifikačních analýz jsme zjistili, že lepší přežití při samotné kompresi hrudníku lze zřejmě přičíst časnému rozdílu v přežívání, který přetrvává v průběhu následujících let sledování. Celkový benefit přežití byl patrný díky použití řady různých statistických srovnávacích testů a multivariačních analýz, které ukazují, že pozorovaný rozdíl mezi oběma populacemi je skutečně robustní. Také jsme si všimli, že rozdíly v přežití pozorované v časné fázi nebyly po dimisi dále zvýrazněny tak, jak jsme předpokládali, ale křivky přežití obou skupin byly spíše podobné. Tyto hlavní nálezy jsou důležité, neboť použití náhradních krátkodobých měřítek prognózy v resuscitačním výzkumu je často v rozporu s mnohem smysluplnějšími dlou- Dumas a kol. Zlepšená perfúze při samotné kompresi hrudníku je prospěšnější u těch nemocných, kdy je patogeneze zástavy kardiální, kdy je nutná defibrilace a u nemocných s krátkodobým kolapsem.5,33–35 Důležité je, že jsme nepozorovali žádnou známku zhoršení stavu u pacientů, u kterých oxygenace a ventilace měla být teoreticky důležitější, tzn. při nekardiální patogenezi zástavy a u srdeční zástavy beze svědků (obrázek 3). Studie má své limitace. Původní studie byly vedeny nezávisle a nebyly navrženy ke zjišťování dlouhodobých výsledků. Běžná vstupní kritéria a randomizace předcházely předpojatosti a umožnily kombinovat studijní data. Každá studie použila národní a provinční registr úmrtí ke zjištění mortality. Adjustace studie neovlivnila výsledky a ty byly obdobné v obou studiích. Přestože jsme pozorovali rozdíly v přežití, nedokázali jsme během dlouhodobého sledování zjistit kvalitu života pacientů, i když víme, že dobrý funkční stav a kvalita života jsou obecně spojovány s lepší dlouhodobou prognózou v celkové populaci.36 Ze studie byli vyloučeni dětští pacienti a ti, u kterých příčinou srdeční zástavy bylo trauma, asfyxie nebo utonutí. Výsledky studie se tedy nedají aplikovat na malý počet jedinců se zástavou v těchto klinických a demografických skupinách. Umělé dýchání bylo prováděno v poměru 2 vdechy k 15 kompresím dle studijního protokolu. Dalo by se předpokládat, že by rozdíly ve výsledcích byly menší, pokud by byl poměr 2:30. Takový závěr je nejistý vzhledem k neúplnému porozumění mechanismům odpovědných za úspěch KPR.37 Přestože jsme zkoumali při kombinaci 2 studií velký počet pacientů, výzkum byl stejně limitován hodnocením podskupin nebo konečných rozdílů ve specifických fázích intervence. Výsledky jsou specifické pro laickou KPR řízenou dispečerem. Poskytují užitečné podklady pro výcvik laické KPR a pro tvorbu doporučení. Je zřejmé, že pro laika je obtížné provést účinné umělé dýchání i po příslušném výcviku.38 Proto naše výsledky podporují doporučení z roku 2010, kde je preferována samotná komprese hrudníku bez ohledu na výcvik nebo asistenci dispečera. Laici jsou vedeni k zahájení umělého dýchání pouze tehdy, jsou-li v provedení této techniky velmi zdatní. Závěrem tedy, naše výsledky jasně ukazují dlouhodobé zlepšení mortality u dispečerem řízené KPR u dospělých pacientů se srdeční zástavou, pokud se strategie KPR omezí pouze na kompresi hrudníku ve srovnání s KPR s umělým dýcháním. Dispečeři záchranné služby mají zásadní úlohu při resuscitaci. To by mělo být bráno velmi vážně, aby došlo ještě ke zlepšení časného rozpoznání zástavy a zefektivnění laické KPR, což by zlepšilo dlouhodobé přežití nemocných se srdeční zástavou. Laická veřejnost může resuscitovat pouze kompresí hrudníku s vědomím, že tato strategie zajišťuje lepší dlouhodobé přežití. Bohužel většina pacientů, kteří již v minulosti prodělali srdeční zástavu, nebyla před příjezdem profesionálních záchranářů laicky resuscitována. Ve srovnání s kombinací komprese hrudníku a umělého dýchání je samotná hrudní komprese strategií KPR, která zjednodušuje psychomotorické nároky a umožňuje snadnější výcvik a mnohem širší použití. Nicméně dlouhodobé přežití při použití samotné hrudní komprese proti kombinaci hrudní komprese a umělého dýchání při laické KPR v běžných terénních podmínkách je nejisté. Naše studie převzala 2 500 případů srdečních zástav ze 2 klinických studií, kde byla provedena dispečerem řízená KPR. Hodnotili jsme, zda se liší dlouhodobá prognóza při použití samotné komprese hrudníku ve srovnání s kombinací hrudní komprese a umělého dýchání. U nemocných resuscitovaných pouze kompresí hrudníku bylo 10% relativní zlepšení přežití ve srovnání se strategií komprese hrudníku a umělého dýchání. Výsledky jasně ukazují dlouhodobé zlepšení mortality u dispečerem řízené KPR při použití strategie samotné komprese hrudníku oproti standardní KPR u dospělých pacientů se srdeční zástavou. Dispečeři záchranné služby mají při resuscitaci zásadní úlohu, ta by měla být využita ke zlepšení časného rozpoznání zástavy a zefektivnění laické KPR, což by zlepšilo dlouhodobé přežití nemocných se srdeční zástavou.
